Complaints - Grievances
If you have a complaint, grievance or would like to report an incident regarding the services we provide or feel you have been discriminated against in any way, you may file a formal complaint.
This may be done by either calling our administrative office at (603) 679-2201 and asking for the Operations Director, or by clicking on the “Complaint/Grievance Form”.
You can print and mail this form to the address listed on the form.
